Clear clinical communication: explaining risk in plain language

Natural frequencies, teach-back and short written summaries dramatically improve patient understanding.

Patients consistently overestimate benefit and underestimate harm when risk is expressed in relative terms. A thirty per cent reduction sounds decisive; three cases avoided in a thousand sounds different.

Use natural frequencies. Saying that four people in a hundred will experience an outcome is understood far more reliably than a percentage change.

Confirm understanding with teach-back: ask the patient to explain the plan in their own words. This identifies misunderstanding without implying criticism.

Write down three lines at the end of the consultation: the diagnosis, the plan, and what should prompt urgent contact. Most of a consultation is forgotten within a day.

In multilingual settings, offer the summary in the patient's stronger language. Comprehension, not politeness, is the objective.
