A simple sequence for mapping a process, removing avoidable steps and testing a change safely.
Most operational problems in clinics are not caused by a lack of effort. They are caused by processes that grew step by step without anyone ever looking at the whole path.
Start by mapping the real process, not the official one. Walk the pathway with the people who perform it and record every handover, wait and duplicate entry.
Then quantify: how long does each step take, how often is it reworked, and where does the patient wait? Two or three measurements are usually enough to expose the bottleneck.
Change one thing at a time and test it on a small scale before spreading it. A two-week trial in one clinic reveals unintended consequences that a policy document never will.
Finally, make the improvement visible. Teams sustain changes they can see working; they abandon changes that disappear into a report.
