Building a living-donor kidney transplant programme across borders

What it takes to establish transplantation between a European centre and a partner hospital in Africa.
Transplantation is the best treatment for most patients with kidney failure, and living-donor transplantation is often the only realistic route where deceased-donor programmes do not exist.
A cross-border programme requires more than surgical expertise. It requires a legal framework for donation, an ethics process that protects donors, tissue typing capability and a reliable supply of immunosuppressive medication.
Partnerships between Geneva and Yaoundé have shown that the transferable element is the system: protocols, training, follow-up pathways and shared case review, rather than the visiting team itself.
Long-term donor follow-up is the obligation most often underestimated. A programme that cannot guarantee it should not proceed.
Success is measured in years of graft survival locally, not in the number of operations performed during a visit.
